The disease is spread by mosquitoes, but a person can pass it to others. Although it wasn’t in the game, it’s mentioned in the series’ manga.
I think that the disease was in the series manga, but it wasnt in the game. The closest I’ve seen was in a fan-made trailer.
That means that if you are an avid fan of the series, you would have probably seen a hint at the disease in the series after the game came out. And now the disease has returned in a way that is almost as deadly as the original. I’m not too sure what the disease is, though I have heard that it only works if you live in an area that is near to the mosquito breeding grounds.
